*Escrow officers: be sure to include your demand request along with the form.
- Escrow/Sale Order Form – to request a demand and documents for a sale transaction only. It is preferred that the escrow company submits this form, as the fees will be paid through escrow (upfront fees may apply). Initial any/all documents needed; transfer and cancellation fee MUST be initialed. Click here for Sale Order Form.
*Lenders – be sure to include your own CERT form along with your request.
- Refinance Order Form – to complete an HOA CERT for a refinance or equity line of credit transaction. A copy of the most current insurance DEC page will be included. Fees will be required upfront prior to processing and must be paid in the form of a company check or cashier's check; we do not accept personal checks and/or cash. If you are a homeowner, you may use these forms to request document copies for your own use. Please be sure and indicate that on your payment. Make your check or money order payable to VITCO Properties, Inc. and mail to our office. Payment must be received prior to providing the necessary document. Note: Fees cannot be billed on your HOA dues statement.
